Authorize Your Repair.

If your vehicle was towed in or you weren't able to sign in person, we'll need your repair authorization on file before we can complete your repairs. It's quick — most customers finish in under 5 minutes.

Why this is required

A signed repair authorization gives our shop your written permission to begin work on your vehicle. It's a legal formality — but an important one. We can't begin repairs until it's on file, so completing it quickly keeps your timeline on track.

What you'll need to sign.

  • ·Your name as it appears on the vehicle title
  • ·Year, make, model and VIN of the vehicle (we already have this on file in most cases)
  • ·Insurance company and claim number, if applicable
  • ·Your signature and date

By signing, you authorize Williams Paint & Body Shop to perform repairs as outlined in your final estimate. Your repairs are covered by our lifetime limited warranty.

What your authorization covers.

  • ·Inspection and teardown. You authorize us to inspect and disassemble the vehicle as needed to identify all damage, including hidden damage that can only be found after teardown.
  • ·Insurance communication. If your repair involves an insurance claim, you authorize us to communicate with your insurer about the repair, including documentation and supplement requests.
  • ·Supplements. Additional damage found after teardown may require a supplement — an updated repair scope submitted to your insurer for review. This is a normal part of the collision repair process.
  • ·Deductibles, betterments, and non-covered charges. The customer is responsible for any applicable insurance deductible, betterment, prior damage, unrelated damage, customer-pay items, and any repair charges not paid by the insurance company. Any deductible applied by the insurance company is due when repairs are complete and before the vehicle is released. Williams Paint & Body Shop does not waive insurance deductibles. Coverage decisions are made by your insurance company, not by the shop.
  • ·Timeline and parts availability. Repair timelines can be affected by parts availability, insurer response times, and the repair scope. We keep you updated as your repair progresses.
  • ·Payment. Any deductible or customer-paid balance is due when the vehicle is picked up. A convenience fee of up to 4% applies to credit card payments, including transactions processed as credit (Google Pay, Apple Pay). No fee applies to debit card payments. Our office is glad to review accepted payment methods and answer payment questions before repairs begin.

Submitting a form on this website does not create a repair authorization — repairs begin only after a signed authorization is on file. The signed form itself is the controlling document; ask our office if you'd like anything explained before signing.
